Insurance Companies & Fees

We will gladly file insurance claims with your insurance company if we are contracted with your carrier. Please be prepared to pay all co-pays, deductibles and other services not covered by your insurance company at the time of service. Please note, for all patients with high deductible plans, we will be collecting $100 at the time of service and will bill you for the remainder of fees. Payment is expected within one billing cycle following the first statement being issued. 

Please become familiar with your health insurance plan and what services are covered as every policy is different.
 
Also, please remember to bring your insurance card every time you come to the office.

We participate with most plans within the following network. Please check with our office regarding particular networks or other plans.
